Grant Reporting
Due Dates
Our current grantees for the 2010 Small, Northern New Mexico Health Grant Group (NNMHGG), and Multi-Year Grants are required to submit an interim and final report during the grant cycle. Discretionary Grantees are required to submit a final report only.
The due date for 2010 Small, NNMHGG and Multi-Year Grantees Interim Reports is Friday, July 8th, 2011.
The due date for 2010 Small, NNMHGG and Multi-Year Grantees Final Reports is Friday, January 20th, 2012.
The due date for Discretionary Grants will vary. Please refer to your grant agreement or contact Cecile LaBore, Grants and Office Manager at clabore@conalma.org for the specific due date.
Reporting Forms
Reporting forms will normally be emailed to grantees about a month before the report is due. Currently available forms may also be downloaded below. (Note: Multi-Year Grantee forms will be emailed only.) You may request forms from Cecile LaBore, Grants and Office Manager at clabore@conalma.org. For specific questions on reporting, please contact Michelle Gutierrez, Program Officer at mgutierrez@conalma.org
To download a form, RIGHT-click on the name of the form, and select “open in a new tab.” Then use your brower’s “save” option to save the form to your desktop.INTERIM REPORTS:

Note: Some people have had issues using the reporting forms. If you experience difficulties, you can try some of these solutions:
- First, make sure you have the most current version of Adobe Reader installed on your computer.
- Second, try using a “save as” with a different name, saving the document to your desktop.
- Third, try “printing” the document as a .pdf and saving it under another name (sometimes your computer will tell you it can’t do that, but then it does it anyway.)
- Fourth, try calling for help! Cecile LaBore, CAHF Information Manager (505-438-0776, ext. 5) will be happy to help you troubleshoot.
- Finally, if all else fails, write your report in a Word document, using the blanks and question numbers on the form, and submit the Word document for your report narrative.
