Photo, Video and Story Release Form

If your organization uses photos,videos or stories of clients, staff or volunteers in your marketing materials, it’s always a best practice to have individuals fill out a release form. We’ve attached the one that we use at the Foundation.  Feel free to use it or modify it for your organization.
