Con Alma Health Foundation is partnering with the Rural Health Public-Private Partnership (RHPPP) to help support the Rural Health Aligned Funding Initiative (Aligned Funding Initiative) focusing on care coordination. Approximately 17 partner foundations are interested in learning about applicants in their service areas who are applying for funding.

The Rural Health Care Coordination Program is now accepting applications! Subject to availability of funds, a total of $7.5 million in funding will be available for up to 10 awards to support rural health consortiums organized to improve patient care and disease outcomes using care coordination strategies. Applicants are required to coordinate the health care delivery services in rural communities through the following four focus areas of collaboration, leadership and workforce, improved outcomes, and sustainability.

How to Apply

Applications for the program will be accepted through March 12, 2020.

Applicants of the Rural Health Care Coordination Program have the opportunity to build public-private partnerships into their application by participating in the Rural Health Aligned Funding Initiative. To do this, applicants are instructed to send their program abstract to participating foundation(s) that serve their area for consideration.

Participating Foundations

To participate in the Aligned Funding Initiative, interested care coordination grant applicants should send their program abstract to participating foundations that serve their area for consideration. Please note that sending a copy of your program application abstract to a RHPPP organization for consideration is non-binding — neither the applicant nor the foundation is required to work together.
