In 2011, Con Alma Health Foundation began gathering information for a new survey of health-related needs and issues to replace the 2006 Report “Closing the Health Disparity Gap” as a road map for grantmaking.

Health Equity in New Mexico: A Road Map for Grantmaking and Beyond

The final version of the report will be issued this year, preliminary documents are already available. Together, they provide a detailed picture of the priorities that will guide the Foundation’s grant making in 2012.

The nine-page Executive Summary includes discussion of all the major priorities and issues and addresses the grant-making considerations that will be applied in relation to each one. This is the key document to review in order to make certain your application will be competitive in the 2012 funding cycle.

The four-page Key Findings and Recommendations document is less detailed, but provides a quick overview for discussion.

2012 NNMHGG Application Guidelines

“In May 2002, the Los Alamos Medical Center was sold by Banner Health Systems, a non-profit corporation, to Province HealthCare, a for-profit corporation. Some proceeds from the sale of Los Alamos Medical Center (LAMC) were preserved to serve the unmet healthcare needs of the people of Los Alamos, Rio Arriba and northern Santa Fe counties.”

*Please note that Northern Santa Fe County is defined as towns and pueblos north of the city of Santa Fe and does not include the city of Santa Fe.

NNMHGG does not fund capital campaigns, annual fund-raising campaigns, endowments, event sponsorships, clinical research, scholarships, routine operational costs, lobbying, media-based programs or campaigns, political campaign activities, or direct financial subsidy of health services to individuals or groups.

You may download and review the Guidelines (link below) to determine whether the goal of your application is likely to meet all requirements. The more closely your project or proposal aligns with the Guidelines and Priorities, the more competitive it is likely to be in this year’s Grant Cycle.

In 2012, Con Alma Health Foundation has allocated $125,000 to fund proposals through the Small Grants funding program.

Before looking at the more detailed Guidelines document, we recommend that you take the Eligibility Quiz to determine whether your organization is an appropriate funding match.

If you pass the Quiz, please download and review the Guidelines (link below) to determine whether the goal of your application is likely to meet all requirements. The more closely your project or proposal aligns with the Guidelines and Priorities, the more competitive it is likely to be in this year’s Grant Cycle.
